Student Commitment & Credit Enrollment Form

For high school students participating in the MIT Internship Program who wish to register with West Nottingham Academy solely for academic credit associated with the internship. This is a limited credit enrollment and is not a transfer of schools.

Program: MIT Internship Program

I wish to participate in the MIT Internship Program and register with West Nottingham Academy solely for academic credit associated with my participation. I understand that this registration DOES NOT constitute a transfer to West Nottingham Academy, and West Nottingham Academy will not become my primary school or school of record as a result of this registration. I will remain enrolled at and responsible to my current school for all attendance, academic, graduation, financial, conduct, and other requirements established by that school.

Participation in the MIT Internship Program and registration for credit at West Nottingham Academy does not replace, waive, reduce, or otherwise alter any obligation I have to my current school. I understand that successful participation in the program may earn a maximum of 1.0 West Nottingham Academy high school credit per academic year. Credit is awarded by West Nottingham Academy upon satisfactory completion of program requirements and verification of participation.

By signing below, the student and parent/guardian acknowledge that this is a limited credit enrollment only, not a transfer to West Nottingham Academy, and that the student remains fully responsible for meeting all requirements of the student’s current school.
